Civilians caught up in Sudan's civil war have given graphic accounts to the BBC of rape, ethnic violence and street executions. Our journalists have managed to make it to the front line of the fighting close to the capital, Khartoum.

Top UN officials have said the conflict has plunged the country into "one of the worst humanitarian nightmares in recent history" and could trigger the world's largest hunger crisis.

There are also fears that in Darfur, in the west of the country, a repeat of what the US called genocide 20 years ago may be beginning to unfold.

The US secretary of state has flown to the Middle East to discuss a post-war plan to govern and secure Gaza.

Antony Blinken's talks with Arab leaders in Saudi Arabia and then Egypt will focus on what the US calls "an architecture for lasting peace".

It comes as witnesses said Israeli forces had escalated their operation around al-Shifa hospital in Gaza City, carrying out a number of air strikes.

Earlier, Israel's military said it had killed 90 gunmen there since Monday.

Separately, indirect negotiations between Israel and Hamas are continuing in Qatar to bring about a ceasefire and the release of hostages. But there are few signs that a breakthrough is imminent.

    Having recently  a close family member treated for this terrible disease I wish Kate well  and everyone else who are suffering at the moment

    The shock of discovery,is something you never forget,the mind goes to places you never knew existed. I wish everybody well who is touched by cancer.
    Politicians and nappies must be changed often,and for the same reason

    The attack on a Moscow concert hall on Friday was the worst in Russia for years. More than 130 people were killed as gunmen stormed the complex, days after President Vladimir Putin began his fifth term of office. The Islamic State group (IS) has said four of its members carried out the attack.[/size]

    This is what is known so far.
    How did the attack unfold?

    Crocus City Hall, on the outskirts of Moscow - 20km (12 miles) from the Kremlin - was about to host a concert by the rock group Picnic when gunmen burst into the foyer just after 20:00 (17:00 GMT) on Friday evening.

    Video showed at least four people shooting randomly before proceeding into the concert hall itself and opening fire there.

    Inside the auditorium, a woman said she and other visitors rushed towards the stage as soon as they realised shots were being fired.

    "I saw a person in the stalls with a sidearm and there were cracks [of gunfire] going off, I was trying to crawl behind a loudspeaker," she told Russian TV.

    At some point, fire can be seen inside the hall. Flames later engulfed the facade, while glass on the top two floors of the seven-storey building blew out.

    "The terrorists used a flammable liquid to set fire to the concert hall's premises, where spectators were located, including wounded," Russia's Investigative Committee said.

  • Helicopters were brought in to drop about 160 tonnes of water, but it took some 10 hours for the fire to be contained.

    By the time the suspects escaped - according to some accounts, the attack lasted about 20 minutes - hundreds had been killed and injured.

    Many died from bullet wounds, some as a result of smoke inhalation.

    Picnic's band members themselves were unharmed.
    Who are the Crocus City Hall victims?

    More than 6,000 Russians had flocked to the retail and concert complex for the event.

    The number of those who were killed has been rising steadily.

    By Sunday afternoon, at least 137 had been confirmed dead. The first official list of casualties suggested the oldest victim was in her 70s, with three children among the dead.
